There will be a 20% charge for no-shows or cancellations within 24 hours of the scheduled appointment. 15 minutes grace period allowed if running late, after such period we reserve the right to reschedule your appointment and charge cancellation fee of 20% of the service charge. Surcharge of $25 to foreign fills (Lash extension sets not done by me). There will be no refunds given on services rendered. You agree not to initiate credit card chargeback disputes. For clients with package(s), when choosing to cancel a package before completion, the credit from the previous services will be reverted as À la carte and refund will be issued for remaining balance minus services rendered. Packages will have a NO REFUND POLICY after 2 months of inactivity. For lash package(s) beyond time limitations of a fill,(3wks) there will be additional charge for full set $30 starting, additional on top of the signed credit. Please expect refunds to take 5-7 business days in the original form of payment, unless otherwise specified. Revitalize Beauty llc reserves the right to refuse service at any time. 7/19/24
